The airplane has exceeded its critical angle of attack, and one wing has stalled more than the other. That asymmetrical stall drives the rotation. One wing generates some lift while driving forward; the other is fully stalled and dropping. The aircraft falls through the sky while rotating around its vertical axis.
A spiral dive is fast and loads the airframe. A spin is slow, the nose is down, and the airplane is autorotating. Pilots who confuse the two will apply the wrong recovery inputs.

What Does a Pattern Stall-Spin Look Like in Practice?
The NTSB has studied this extensively. Loss of control in flight is the leading cause of general aviation fatalities, year after year. Within that category, the stall-spin accident - particularly in the traffic pattern - is consistently one of the top killers.
Consider a standard scenario: flying a Cessna 172, right-hand pattern to runway 14, winds 180 at 12 knots. On base leg, a crosswind is pushing the aircraft inside toward final. The pilot shallows the bank and adds right rudder to bring the nose around without steepening the turn.
